Nature and Scope

Under the direction of the Director of Nursing, the Social Service Designee is responsible for the coordination and direction of the social service program of the Skilled Nursing Facility (SNF).  Identifies the medically related social and emotional needs of the resident and arranges for the meeting of these needs through individual and group social service programming and appropriate referrals to outside social service agencies, community resources and professional services.

Compensation

Compensation Philosophy:

  • We have 30 pay increments on our pay scales to compensate staff for 30 years of relevant experience. 
  • Social Service Designees who  possess a Certified Nursing Assistant (CNA) certification earn a higher rate of pay.

Compensation range:

  • $20.39 - $29.07 without a CNA
  • $23.46 - $33.45 with a CNA

Qualifications

Experience:

  • Prefer long term care experience and use of Point Click Care for charting

Education: 

  • Social Service Designee Certificate within three (3) months of hire.
  • Prefer CNA and current certification in CPR.

Knowledge, Skills, and Abilities:

  • Shows adaptability to varied situations, is empathetic, enthusiastic, creative and a resourceful personality
  • Microsoft 365 programs such as Word, Excel, and Powerpoint
  • Possesses good interpersonal skills, works well independently and with supervision, communicates well and is able to problem solve.
  • Possesses strong psychosocial assessment skills, has good interview skills and is knowledgeable regarding community resources.
  • Fulfills responsibility to the facility by adhering to its philosophy and general policies.
  • Is able to communicate in English both verbally and in writing.

Responsibilities

  • Assuring that the social service needs of each resident are assessed on admission and quarterly thereafter and that an individualized plan of care is developed and carried out related to his/her psychosocial concerns.
  • Completes the psychosocial sections of the MDS form in a timely manner and care plans.
  • Assures that progress notes are maintained on each resident specific to his/her social service needs that interventions address the needs and documentation reflects progress or lack of progress toward achieving the specific goal.
  • Keep all departments aware of upcoming appointments and needs required from those departments.
  • Interview and written assessment of each patient after admission to the service.

Child Care:

  • PDH is thrilled to provide our Child Care Center services to all families in our area. Our program is operational from 7:45am to 5:15pm, Monday through Friday, and is approved for children aged 0 to 5 years. After submitting your application, our Child Care Director will get in touch with you about your acceptance into the program. Once approved, we will start the enrollment process.
