Nature and Scope

We are excited to open our Skilled Nursing Facility and plan to start admitting patients in the summer of 2025. While we aim to fill positions as soon as possible, please note that start dates may be adjusted if the opening schedule changes. We currently expect this position to begin in late June or early July 2025.

 

The Minimum Data Set (MDS) Coordinator works in conjunction with physicians, other nursing staff, residents, families and other facility departments to assure that the resident assessment tool, resident care plan and interdisciplinary team meeting are completed according to all Federal/State regulations and according to facility policy/procedure in order to provide optimum individualized nursing care for the residents of SNF service.

Compensation

Compensation Philosophy:

  • We have 13 pay increments on our clinical pay scales to compensate staff for 30 years of relevant experience. 

Compensation range:

  • $55.77 - $79.52

Qualifications

Education:

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university
  • Must possess a RN California License
  • Current CPR

Knowledge, Skills, and Abilities:

  • Working the floor and performing daily RN duties.
  • Ideal candidate must be a skilled communicator, director, and motivator
  • Able to organize and prioritize many tasks effectively

Must meet physical requirements as stated on job description addendum

 

 

Responsibilities

  • Completes the initial MDS assessment on new admissions, annual, and quarterly reassessments and change in status assessments within required time frames. Obtains data from a variety of sources to assure accuracy and completeness and documents same according to Federal, State, and facility guidelines.
  • Follows resident assessment protocols to assure that all areas in the assessment process are considered and care planned accordingly. Documents decision making process according to facility policy.
  • Completes the care plan for all new admissions within required time frames and assures that all resident problems are addressed with specific and resident oriented and with appropriate interventions.
  • Coordinates the interdisciplinary team meeting and assures that all disciplines as well as the resident and/or family have input into the resident’s plan of care. Update care plan accordingly and assures that problems have been appropriately followed-up on.
  • Enters all assessment information into the computer and develops the care plan according to data entered. Is knowledgeable regarding the computer capabilities and maximizes its use.

Child Care:

  • PDH is thrilled to provide our Child Care Center services to all families in our area. Our program is operational from 7:45am to 5:15pm, Monday through Friday, and is approved for children aged 0 to 5 years.
